Description
Emerging in the posterior neck, the lateral semispinalis capitis is presented deep to the splenius capitis and trapezius, with its fibers running superomedially from the transverse processes of the upper thoracic and mid cervical vertebrae toward the occipital bone between the superior and inferior nuchal lines. Medially, it blends into the broader semispinalis capitis mass adjacent to the spinous processes and the nuchal ligament, while laterally it is bounded by the more superficial splenius and the posterior border of the sternocleidomastoid region. Inferiorly, the muscle continuity is seen with the cervical portion of the erector spinae group; superiorly, it approaches the suboccipital region beneath the scalp. Semispinalis capitis matters because it is one of the main posterior extensors and contralateral rotators of the head, and it becomes clinically relevant in cervicogenic headache and post whiplash pain patterns. The greater occipital nerve (dorsal ramus of C2) typically courses through or near semispinalis capitis before reaching the scalp, so hypertrophy, spasm, or fibrotic change in this layer can contribute to occipital neuralgia symptoms and guides both palpation-based diagnosis and targeted injection strategies. A key landmark.
