Emerging bilaterally from the posterior cervical region, the lateral portion of semispinalis capitis runs superomedially from the upper thoracic and lower cervical vertebrae toward its broad insertion on the occipital bone between the superior and inferior nuchal lines. Posteriorly, the external occipital protuberance and nuchal ligament provide midline landmarks, while the muscle bellies sit deep to the splenius capitis and trapezius and lateral to the midline semispinalis capitis fibers. Inferiorly, the layered extensor mass blends with semispinalis cervicis over the cervical spinous processes, with the cervical vertebrae and portions of the scapulae framing the lower field. Depth is the point. Semispinalis capitis is a primary generator of cervical extension and contralateral rotation, and its segmental innervation by dorsal rami makes it a common target in cervicogenic headache and posterior neck pain patterns. Trigger points in the upper fibers can refer pain to the occiput and vertex, overlapping with greater occipital nerve distribution, which is why clinicians often palpate and inject in this compartment with attention to depth and midline. For posterior cervical approaches, appreciating its relationship to splenius capitis helps explain why deeper dissection changes the line of pull and exposes the short suboccipital muscles near the skull base.
