Description
Layered posterior musculature is presented from the occiput to the calves, with the trapezius spanning the superior thorax and nuchal region and the deltoid capping the lateral shoulder. Deep to the superficial back sheet, the longissimus capitis runs superiorly from the cervical and upper thoracic transverse processes toward the mastoid process, lying lateral to the spinous processes and medial to the more lateral longissimus cervicis and iliocostalis bundles. Inferiorly, the latissimus dorsi forms the posterolateral trunk contour above the gluteus maximus, while the hamstring group occupies the posterior thigh and the gastrocnemius forms the superficial calf. Longissimus capitis matters because it is a primary extensor and ipsilateral rotator of the head and neck, and its deep position alongside the cervical articular pillars places it in the middle of common pain generators after whiplash and prolonged forward-head posture. Palpable hypertonicity in this muscle, and in adjacent semispinalis capitis and splenius capitis, often contributes to cervicogenic headache patterns that refer toward the mastoid and occiput, and it also limits rotation during clinical range-of-motion testing. Dorsal rami of cervical spinal nerves supply it, a useful reminder when localizing posterior neck symptoms versus trapezius-related pain from the spinal accessory nerve.
