Description
Running longitudinally along the posterolateral cervical spine, the longissimus capitis is visible as a thick column of erector spinae fibers ascending from the upper thoracic and lower cervical transverse processes toward the mastoid region of the temporal bone. Medial to it, the semispinalis capitis forms a broader, more midline mass that spans toward the occipital bone, while the longissimus cervicis continues inferiorly as a narrower tract between the cervical transverse processes. Superficial layers of the posterior trunk and shoulder girdle, including trapezius, posterior deltoid, and latissimus dorsi, frame the neck and upper back, and the scapulae sit laterally with the infraspinatus occupying the infraspinous fossae. The erector spinae columns flank the spinous processes from thorax into the lumbar region. Strong lines, clear landmarks. Longissimus capitis is a workhorse extensor and ipsilateral rotator of the head and neck, and its fiber direction makes it easier to explain how posterior cervical muscle imbalance can drive limited rotation after whiplash or prolonged forward head posture. Palpation and dry needling targets often sit just lateral to the semispinalis capitis and deep to splenius capitis, a relationship that is hard to teach without a clean posterior dissection-style view. Posterior approaches to the cervical spine also benefit from recognizing where the deep extensor mass lies relative to the cervical transverse processes and facet joints, where the medial branches of the dorsal rami are commonly addressed for radiofrequency ablation in facet-mediated neck pain.
