Description
Running longitudinally on either side of the thoracic spinous processes, the longissimus thoracis appears as paired muscular columns within the erector spinae mass, positioned lateral to spinalis and medial to iliocostalis. Posteriorly, its fibers are seen spanning between the thoracolumbar fascia inferiorly and the posterior ribs and thoracic transverse processes superiorly, creating a continuous paraspinal contour. Superficial back muscles rendered in gray, including trapezius across the cervicothoracic junction and latissimus dorsi over the lower thorax, frame the deeper red-highlighted extensors. Clinically, this posterior perspective clarifies the layered relationships that matter in both examination and intervention: tenderness in the paraspinal gutter often localizes to the erector spinae rather than trapezius, and palpation just lateral to the spinous processes tracks the longissimus more reliably than the more lateral iliocostalis. Segmental activation and spasm of longissimus thoracis commonly contributes to mechanical thoracic back pain and exaggerated lumbar lordosis patterns, and it is a frequent target for dry needling, trigger-point injection, or EMG needle placement where staying medial avoids the costal angles and pleura.
