Description
Running longitudinally along the posterior thoracolumbar trunk, the longissimus thoracis is shown as the intermediate column of the erector spinae, positioned lateral to spinal midline and medial to the more superficial latissimus dorsi plane one would typically reflect to expose it. Superiorly, its slips course alongside the thoracic vertebrae and fan toward the ribs, while inferiorly the muscle belly thickens toward the lumbar region, blending with the common erector spinae tendon and fascial attachments near the posterior iliac crest and sacrum. The ribcage, thoracic and lumbar vertebral column, and pelvis provide the bony frame, with the longissimus thoracis spanning from the lumbosacral region toward the upper thorax. Orientation is posterior. Longissimus thoracis matters when you want to distinguish true paraspinal musculature from more superficial back extensors and understand the mechanical line of pull that produces trunk extension and ipsilateral lateral flexion. In posterior approaches to the thoracic and lumbar spine (for example, exposure for laminectomy or instrumentation), the muscle is commonly detached or split from its spinous and transverse process neighborhood, a step that correlates with postoperative paraspinal denervation, atrophy on follow-up imaging, and persistent axial back pain. This view also supports teaching of erector spinae compartmentalization, separating longissimus from spinalis medially and iliocostalis laterally, which helps interpret focal tenderness patterns and trigger points in paraspinal strain. Ideal for gross anatomy lab manuals, orthopaedic and neurosurgical atlases describing midline posterior spine exposure, and rehabilitation texts discussing erector spinae strengthening and motor control in low back pain.
