Description
Posterior to the flexed knee, the medial and lateral heads of the gastrocnemius sweep inferiorly from their femoral origins to form the superficial contours of the popliteal region, their muscle fibers converging toward the calcaneal (Achilles) tendon distally. Deep to them, the popliteus runs obliquely from the lateral femoral condyle and lateral meniscus to the posterior tibia, sitting superior to the soleal line and immediately adjacent to the posterior capsule. Medially, the semimembranosus tendon broadens toward its insertion on the posteromedial tibia and expansion to the oblique popliteal ligament, while laterally the biceps femoris tendon approaches the fibular head; the tibia and femur remain visible at the joint line, capped by articular cartilage where the posterior condylar surfaces approximate in flexion. Fascial planes and tendinous bands frame the joint. Clear landmarks. Knee flexion relaxes parts of the posterior capsule and shifts the gastrocnemius away from the joint line, which makes this posterior perspective useful for teaching the layered anatomy encountered in posterior approaches and in ultrasound scanning of the popliteal fossa. That relationship matters when distinguishing a Baker cyst (gastrocnemius semimembranosus bursa) from other posterior knee masses and when correlating posterolateral corner injuries with nearby tendon and capsular structures. Popliteus orientation is also a teaching point for internal rotation of the tibia during early flexion and for meniscal attachment at the lateral compartment.
