Description
Arising from the distal femur and spanning to the proximal tibia, the anterior cruciate ligament and posterior cruciate ligament cross obliquely in the intercondylar notch, a relationship made clearer with the knee held in flexion. Along the medial and lateral margins, the tibial (medial) collateral ligament and fibular (lateral) collateral ligament run longitudinally as extra-articular stabilizers, with the fibular collateral ligament positioned more laterally toward the fibular head. Posteriorly, the joint capsule is reinforced by the oblique popliteal ligament sweeping superolaterally from the semimembranosus expansion and the arcuate popliteal ligament curving over the popliteus region near the posterolateral corner. No patella is seen. Flexion tightens the PCL and reorients the cruciates, which is why a posterior view in this position reads like a functional map of anteroposterior restraint rather than a static anatomy plate. That matters when correlating mechanism of injury and exam, the classic posterior drawer test and the tibial sag sign assess PCL integrity with the knee flexed, while combined injury patterns at the posterolateral corner can leave the PCL graft at risk if varus and external rotation laxity are missed. Arthroscopic tunnel placement and open posterolateral reconstructions also depend on clear spatial understanding of the intercondylar region relative to the posterior capsule and popliteus complex.
