Description

Viewed from the medial side with the knee flexed, the distal femur articulates with the proximal tibia while the medial femoral epicondyle and medial tibial condyle frame the medial compartment. The tibial collateral ligament (medial collateral ligament) spans from femur to tibia, with its deep fibers blending with the joint capsule and anchoring to the medial meniscus along the joint line. Posteriorly, the oblique popliteal ligament can be appreciated as a capsular thickening sweeping superolaterally from the semimembranosus expansion toward the lateral femoral condyle. The pes anserinus tendons course anteromedially toward their insertion on the proximal medial tibia. Stability lives here. Flexion changes what you can teach and what you can injure. With the knee bent, the superficial and deep portions of the MCL separate conceptually and mechanically, and the meniscofemoral and meniscotibial attachments become easier to relate to valgus instability patterns. Clinically, a valgus force with external rotation can sprain the MCL and secondarily stress the medial meniscus, a linkage that underlies medial joint line tenderness after a grade I to II MCL injury and influences bracing versus surgical decision-making. Use this artwork in gross anatomy and musculoskeletal modules to explain medial knee stabilizers, in sports medicine teaching files when correlating exam maneuvers such as the valgus stress test at 30 degrees, and in orthopaedic or radiology publications discussing MCL injury grading on MRI and associated meniscal pathology.
