Description
Arising from the medial margin of the patella, the medial patellotibial ligament (MPTL) courses inferiorly and slightly medially to blend with the anteromedial tibia near the medial tibial condyle, just distal to the joint line. An anterior perspective also places the patella within the extensor mechanism, continuous superiorly with the quadriceps tendon and inferiorly with the patellar ligament inserting on the tibial tuberosity. Medially, the superficial medial collateral ligament runs more vertically from the medial femoral epicondyle toward the proximal tibia, while deeper in the intercondylar region the anterior cruciate ligament and posterior cruciate ligament sit behind the tibial plateau. Menisci cap the tibial plateau. Articular cartilage highlights the femoral condyles, tibial plateau, and patellar facets. Clinical attention to the MPTL is tied to patellar instability, where medial soft-tissue restraints fail after lateral patellar dislocation and the patella translates laterally relative to the trochlea. This anterior view clarifies how the MPTL differs from the medial patellofemoral ligament (MPFL): the MPFL runs from patella to medial femur, whereas the MPTL anchors the patella to the tibia and can be targeted in distal realignment or reconstruction strategies, especially when addressing abnormal tilt or translation with knee flexion.
