Patellar Ligament, Anterior View

Description

Centered on the anterior knee, the patellar ligament (patellar tendon) descends from the apex of the patella to the tibial tuberosity, forming a broad, striated band in the midline just inferior to the patella. Medial and lateral to it, the medial and lateral patellar retinacula blend with the joint capsule and flank the infrapatellar region, while the patella sits anterior to the distal femur, separated from the femoral trochlea by the patellofemoral joint space. Superiorly, the quadriceps tendon continues into the patella, completing the extensor mechanism. Articular cartilage caps the distal femoral condyles and tibial plateau at the margins of the joint line. Anterior orientation matters here because it clarifies the extensor apparatus as a continuous kinetic chain from quadriceps to tibia, and it makes the tibial tuberosity a clear distal landmark for clinical palpation and procedural planning. Patellar tendinopathy (jumper’s knee) most often localizes at the proximal patellar ligament near the inferior pole of the patella, whereas Osgood Schlatter disease targets the tibial tuberosity apophysis in adolescents. A long, uninterrupted view of the ligament also supports teaching safe portal placement and avoidance zones in anterior knee arthroscopy, where superficial structures and the anterior capsule can be at risk.
