Description
Seen from the medial aspect, the patellar ligament (patellar tendon) runs inferiorly from the apex of the patella to the tibial tuberosity on the anterior proximal tibia, forming the anterior anchor of the knee’s extensor mechanism. Medial to the ligament, the medial femoral condyle and medial tibial condyle frame the tibiofemoral articulation, with a wedge of medial meniscus interposed between their articular cartilage surfaces. The patella sits anterior to the distal femur, and the patellar ligament lies superficial to the joint capsule, with adjacent medial retinacular fibers blending toward the anteromedial tibia. Clear landmarks. A medial view matters because it clarifies how the patellar ligament aligns with the tibial tuberosity and the anteromedial tibial surface, a relationship that is easy to misread in anterior-only diagrams when teaching patellofemoral tracking and extensor lag. Clinically, this orientation supports discussion of patellar tendinopathy at the inferior pole of the patella, tibial tuberosity apophysitis (Osgood-Schlatter disease), and rupture scenarios where the patella rides proximally (patella alta) and active knee extension fails. It also complements surgical planning for anterior knee approaches and graft harvest, where respecting the patellar ligament and its paratenon affects postoperative pain and healing.
