Description
Medially oriented anatomy of the knee joint is presented with the distal femur superiorly and the proximal tibia inferiorly, framed by the fibrous joint capsule and medial retinacular tissues. The medial patellotibial ligament runs from the inferomedial margin of the patella toward the anteromedial tibia, coursing inferior to the patellar articular surface and lying anterior to the superficial medial collateral ligament along the medial joint line. Articular cartilage on the femoral condyle is rendered in blue, clarifying the relationship between the capsuloligamentous layers and the medial femorotibial compartment. Layering matters. Clinically, the medial patellotibial ligament is part of the medial patellar stabilizer complex that resists lateral translation, working with the medial patellofemoral ligament and medial patellomeniscal fibers across early flexion. This medial aspect view supports teaching and operative planning because it clarifies where the distal medial restraint inserts relative to the anteromedial tibia and capsule, a common source of confusion when discussing medial retinacular plication or tibial-sided soft tissue fixation in recurrent patellar instability. It also helps explain why iatrogenic over-tensioning can increase medial patellofemoral contact pressure and provoke anterior knee pain.
