Description
Positioned in posterior orientation, the heart is viewed from its diaphragmatic and atrial surfaces, bringing the left atrium and the entry of the pulmonary veins into clearer relation with the posterior left ventricle and the inferior aspect of the right atrium near the inferior vena cava. Nerve fibers course over the epicardium toward the atria and along the atrioventricular and interventricular grooves, where they travel in close company with the coronary arteries and veins. Small cardiac ganglia and neural tracts cluster around the base of the heart near the great vessels, then distribute toward nodal tissue at the junctions of the atria. Coronary vascular branches are rendered in red and blue, providing a clean landmark system for the accompanying neural pathways. Posterior cardiac innervation matters because the clinically relevant autonomic inputs to the sinoatrial node and atrioventricular node converge near the venous inflow and the atrial roof, regions frequently manipulated during catheter ablation for atrial fibrillation and during left atrial appendage procedures. Injury or overstimulation of these fibers can present as sinus bradycardia, AV block, or reflex hypotension, and the anatomic proximity to the coronary sinus and posterior left atrial wall is a recurring teaching point for electrophysiology.
