Description

Opening into the left ventricle from the luminal side, the thick left ventricular myocardium forms a high-walled chamber with coarse trabeculae carneae ridging the inner surface. Anteriorly and medially, the left ventricular outflow tract would lead superiorly toward the aortic vestibule and aortic valve, while posteriorly the inflow region relates to the mitral valve orifice. Along the ventricular wall, papillary muscles rise from the trabeculated floor and project toward the valve plane, tethered by chordae tendineae to the mitral leaflets. This is dense muscle. That internal architecture matters because it is where function and pathology become tangible: left ventricular hypertrophy thickens the wall and can narrow the outflow tract, and ischemic injury to a papillary muscle can precipitate acute mitral regurgitation. This luminal perspective also clarifies why mural thrombus after an anterior myocardial infarction often forms in regions of relative stasis between trabeculae, and why endocarditis-related vegetations are sought along the valve apparatus and adjacent endocardium.
