Description
Seen from a posterior perspective, the cervical spinous processes descend from the external occipital protuberance to the cervicothoracic junction, with the superior thoracic vertebrae continuing inferiorly between the medial borders of the scapulae. Over the lower lateral neck, the omohyoid is positioned as a slender infrahyoid muscle with an inferior belly arising near the superior border of the scapula, then coursing superiorly and medially toward an intermediate tendon before continuing as a superior belly toward the hyoid region (anterior structures implied by its trajectory). Lateral to the vertebral column, the scapular spine and the posterior ribs frame the muscle’s oblique line of pull across the posterior triangle of the neck. Orientation of the omohyoid matters because its intermediate tendon is tethered to the clavicle and deep cervical fascia, creating a visible landmark relationship to the internal jugular vein and the lower cervical lymphatic chain during neck dissection. Surgeons use this muscle as a guide when approaching the carotid sheath and levels III to IV lymph nodes, and its course helps explain why a low lateral neck mass can distort the contour of the posterior triangle. Small structure, real consequences. Faculty commonly choose this posterior neck and shoulder-girdle composition for teaching infrahyoid anatomy in gross anatomy lab, reinforcing the distinction between anterior strap muscles and the posterior cervical triangles using bony landmarks students can palpate. It also suits operative anatomy figures for thyroidectomy, selective neck dissection, and central line discussions where omohyoid location relative to the internal jugular region is referenced.
