Description
Originating near the superior border of the scapula close to the suprascapular notch, the inferior belly of the omohyoid runs anteromedially from the posterior shoulder toward the lateral neck, then narrows into an intermediate tendon that typically lies deep to the lower sternocleidomastoid and just superior to the clavicle. From a posterior perspective, the muscle’s course can be appreciated in relation to the scapular body and spine laterally and the cervical spinous processes medially, with the occipital bone and upper thoracic vertebrae providing proximal skeletal context. The superior belly is inferred as it ascends to the hyoid bone in the anterior neck, continuing the characteristic strap muscle line between shoulder girdle and suprahyoid framework. Surgeons care about the omohyoid because it is a dependable landmark in the posterior triangle: the intermediate tendon and inferior belly help orient the dissection toward the internal jugular vein and the lower carotid sheath while the ansa cervicalis contributes motor supply to both bellies. In selective neck dissection, the muscle often marks the level of nodal basins in the lower lateral neck, and its fascial sling to the clavicle can tether the intermediate tendon, a relationship that becomes obvious when the shoulder girdle is elevated or retracted.
