Description
Running obliquely across the anterolateral neck, the omohyoid appears as a paired infrahyoid strap muscle with an inferior belly arising from the superior border of the scapula near the suprascapular notch, an intermediate tendon deep to the sternocleidomastoid, and a superior belly inserting on the inferior border of the hyoid bone. Medial to it lie the sternohyoid and sternothyroid muscles in the visceral compartment, while laterally the carotid sheath structures (common carotid artery and internal jugular vein) occupy the carotid triangle just deep to the superficial fascia and platysma. Superiorly, the hyoid sits beneath the mandible and tongue base; inferiorly, the muscle crosses toward the clavicle and upper thorax in front of the cervical vertebrae and rib cage. Surgeons and trainees use the omohyoid as more than a named strap: its superior belly forms a functional boundary within the anterior triangle, and its intermediate tendon tethers to the investing layer of deep cervical fascia, shaping the fascial planes that guide fluid spread and surgical dissection. During neck dissection for head and neck cancer, the omohyoid is commonly divided or retracted to access levels III and IV lymph nodes, and it helps orient the relationship of the internal jugular vein to surrounding muscles. For central venous catheter placement, appreciating how the sternocleidomastoid overlies the tendon and how the carotid artery runs medial to the internal jugular vein reduces wrong-plane needle passes. Use this anterior perspective for teaching infrahyoid muscle anatomy in gross anatomy or head and neck modules, for ENT and endocrine surgery manuals describing strap muscle retraction in thyroidectomy, and for radiology or ultrasound education where the omohyoid can appear as a thin muscular band crossing the lower neck.
