Description
Seen from a posterior, inferior angle into the male pelvis and perineum, the sacrum and posterior ilia frame the pelvic outlet while yellow peripheral nerves course forward from the sacral plexus. Branches consistent with the pudendal nerve (S2 to S4) track along the lateral pelvic wall toward the ischioanal region, then curve anteriorly in the expected trajectory around the ischial spine and sacrospinous ligament region before entering the perineum. Distally, the anterior scrotal nerves are shown sweeping from a more posterior origin to reach the anterior perineum and scrotal skin, aligning with the distribution of the perineal branches and genital sensory pathways. Orientation matters. Tracing the anterior scrotal nerves from a posterior vantage clarifies a common teaching stumbling block: their cutaneous territory is anterior, but their course begins deep and posterior, adjacent to landmarks used in pudendal nerve blocks and pelvic floor surgery.
