Description
Viewed from posterior in the male perineum, the pudendal nerve is followed distally as it exits the pelvis near the ischial spine, hooks around the sacrospinous ligament, and re-enters through the lesser sciatic foramen to course in the pudendal (Alcock) canal along the medial surface of the obturator internus fascia. Inferior to the pelvic floor, the deep perineal nerve is shown traveling anteriorly in the deep perineal pouch with the internal pudendal vessels, giving branches that run medial toward the external urethral sphincter and lateral toward the perineal musculature. Posteriorly placed branches toward the anal region align with the inferior rectal nerves, while more anterior fibers continue as dorsal nerve(s) of the penis. Key landmarks. Sacrum, coccyx, and the paired ischial tuberosities frame the field. This posterior perspective matters because the pudendal neurovascular bundle is approached and at risk at reproducible bony and ligamentous landmarks, not in abstract “perineal space”. Pudendal nerve block techniques target the nerve near the ischial spine, and pudendal neuralgia often reflects entrapment between the sacrospinous and sacrotuberous ligaments or within Alcock canal, producing perineal pain with possible sensory change of the penis, scrotum, and anal verge. Surgeons also reference this course during posterior approaches to the ischioanal fossa, fistula surgery, and transperineal procedures where deep perineal branches to the sphincter complex and perineal body can be injured.
