Description
Seen from a posterior perspective in the male pelvis, the inferior rectal nerve branches from the pudendal nerve in the ischioanal fossa and courses medially toward the anal canal, where its fine fibers spread across the anal triangle. The sacrum lies midline and posterior, with the ilia flanking it laterally and the coccyx inferior to the sacral apex, providing bony orientation behind the pelvic floor. Yellow-highlighted neural elements trace the sacral plexus on the anterior surface of the piriformis region, then continue toward the pudendal canal, with the inferior rectal branch turning back toward the external anal sphincter and perianal skin. Small branches fan out. Fast. Posterior visualization matters because the inferior rectal nerve is most vulnerable where it crosses the ischioanal fat to reach the external anal sphincter, a corridor shared with vessels and traversed during drainage of ischioanal abscesses and surgery for fistula-in-ano. Loss of its motor supply to the external anal sphincter and sensory supply to perianal skin can contribute to postoperative pain, numbness, or fecal incontinence, so appreciating its relationship to the pudendal nerve and pelvic sidewall is not academic. For regional anesthesia, this pathway also explains why a pudendal nerve block may spare perianal sensation if the inferior rectal branch arises proximally or follows an aberrant course.
