Posteriorly oriented, the basal bronchial tree is traced from the distal main bronchi into the lobar and segmental bronchi that course inferiorly toward the diaphragmatic surface of each lung. Right and left lower lobe bronchi run inferolaterally from the hila, with smaller segmental branches diverging laterally toward the costal surface and medially toward the posterior mediastinal border. Branching density increases distally. Cartilaginous bronchial walls remain evident proximally and taper as the tubes narrow. A rear view of the lower bronchial bifurcations helps clarify how the dependent portions of the lungs are ventilated and drained, where secretions tend to pool in supine or postoperative patients. The segmental pattern is also a practical map for bronchoscopy and for planning an anatomic segmentectomy, since the posterior basal segments of the lower lobes are frequent sites of aspiration pneumonia and can be targeted when disease is localized. Endobronchial obstruction from a mucus plug or tumor is interpreted against this branching logic. Side-to-side comparison matters, because the right bronchial tree is typically wider and more vertical, shaping the distribution of aspirated material.
