Description
Viewed from the left, the bronchial tree at the base of an adult lung is centered on the left main bronchus as it continues inferiorly from the carina and gives rise to lobar and then lower segmental bronchi. Inferior and posterior branches corresponding to the lower lobe segments are visible as they course distally toward the diaphragmatic surface, while more superior segmental takeoffs sit closer to the hilum. Branches diverge laterally toward the costal surface and medially toward the mediastinal surface, establishing the familiar fan of bronchial tubes that defines segmental anatomy. Orientation is clear. Segmental branching at the lung base is where anatomy stops being academic and starts guiding decisions in bronchoscopy, thoracic surgery, and cross sectional imaging correlation. A left sided perspective helps when planning a targeted bronchoalveolar lavage or transbronchial biopsy for basilar disease, since the lower lobe bronchi often angle posteriorly and inferiorly, and misidentification can send instruments into the wrong segment. It also supports teaching of why aspiration pneumonia and dependent atelectasis preferentially involve lower lobe segments, and how mucus plugging in a single segmental bronchus can produce sharply marginated subsegmental collapse on radiographs and CT. Use this illustration in respiratory anatomy and pulmonary medicine courses to teach the hierarchy from mainstem bronchus to lobar and segmental bronchi, and in radiology or thoracic surgery texts to annotate segment based reporting and segmentectomy planning at the lung base.
