Description
Seen from an inferior perspective, the right and left main bronchi enter the pulmonary hila and branch into lobar bronchi, then segmental bronchi that radiate toward the diaphragmatic (basal) surface of each lung. Inferiorly, the basal segmental bronchi (for example, right lower lobe superior and basal segments, and left lower lobe superior and anteromedial, lateral, and posterior basal segments) terminate in a fanlike pattern within the parenchyma. More lateral branches track toward the costal margins, while more medial branches converge toward the hilum, where the bronchial tree lies anterior to the esophagus and flanked by the pleural reflections. Color-coding clarifies branching order. An inferior view is the one you want when teaching how the lower lobes are organized, because most aspiration events and many dependent pneumonias preferentially involve posterior and basal segments in supine patients.
