Description
Seen from a posterior perspective, the scapular spine and posterior surface of the scapula frame the proximal humerus, with the rib cage forming the thoracic backdrop. A nerve branch consistent with the radial nerve descends along the posterior arm, giving a muscular branch to the medial head of triceps brachii on the posteromedial humeral shaft and continuing distally toward the anconeus at the posterolateral aspect of the elbow. Long and lateral heads of triceps sit more superficially and laterally, while the medial head lies deep, close to the humerus. Orientation is clear. That relationship matters because motor supply to triceps and anconeus is a functional readout of radial nerve integrity along the posterior arm, where the nerve tracks in the radial groove and then approaches the lateral intermuscular septum. Weakness of elbow extension with preserved deltoid function, or denervation of the anconeus on EMG, helps localize a lesion distal to the axilla and can distinguish midshaft humeral fracture injury from posterior cord involvement. Surgical approaches to the posterior humerus, and posterior elbow exposure near the anconeus interval, benefit from anticipating where this branch runs relative to the triceps heads and the humeral cortex. Use this asset in upper limb anatomy teaching to clarify how the brachial plexus continues as the radial nerve and distributes to the posterior compartment, and in orthopaedic or trauma publications discussing humeral shaft fractures, radial nerve palsy patterns, or posterior plating corridors.
