Description
Seen from the lateral aspect of a male upper limb, the triceps brachii occupies the posterior compartment of the brachium beneath a semi-transparent skin envelope. The long head courses along the posteromedial arm from the infraglenoid tubercle, while the lateral head dominates the posterolateral surface of the humerus and the medial head becomes more apparent distally as it contributes to the common triceps tendon. All three heads converge inferiorly to a thick tendon inserting on the olecranon of the ulna, immediately posterior to the elbow joint line. A lateral perspective on the triceps matters because it clarifies the relationship between the posterior arm musculature and the distal humerus, where trauma and surgical approaches often intersect. Posterior or posterolateral approaches to the humeral shaft place the radial nerve and profunda brachii artery at risk as they traverse the radial (spiral) groove deep to the triceps, a correlation that is easy to miss when only the superficial contour of the lateral head is considered. This is the working anatomy behind radial nerve palsy associated with midshaft humeral fractures and triceps-splitting exposures.
