Description
Seen from the medial aspect, the posterior compartment of the upper arm is dominated by triceps brachii, with the long head running proximally from the infraglenoid region and descending along the medial side of the humerus toward the olecranon. Distal to the muscle belly, the triceps tendon narrows and approaches the posterior ulna, while the medial epicondyle and trochlear region of the humerus sit anterior and slightly inferior to the triceps mass. The medial head’s fiber direction becomes more apparent deep and distal, tucked against the posterior humeral shaft and emerging around the distal third of the arm. Bone landmarks matter here. A medial perspective clarifies relationships that get lost in posterior or lateral views, especially the long head’s position relative to the shoulder joint and the interval it forms with adjacent medial arm structures during surgical exposure. Clinically, this is the side where you think about safe dissection planes in posterior elbow approaches and where iatrogenic injury can occur if the medial soft tissues are handled aggressively, given the proximity of the ulnar nerve at the medial epicondyle and the distal triceps insertion at the olecranon. It also reinforces why distal triceps tendinopathy or avulsion presents with pain and weakness at terminal elbow extension, with tenderness posterior to the ulna rather than at the radius or humeral condyles.
