Description
Posteriorly oriented anatomy centers on the cystic vein coursing from the gallbladder neck and body toward the portal venous system within the hepatocystic triangle. From this vantage, the vein sits posterior to the cystic duct and closely related to the cystic artery, with the gallbladder bed and visceral surface of the liver lying superior and medial, and peritoneal and connective tissue planes layering between the biliary structures and the hepatic parenchyma. Short venous tributaries may be seen entering the cystic vein from the gallbladder wall, then converging proximally toward the right hepatic lobe side. Small vessel. Tight space. Surgical relevance is immediate. During laparoscopic cholecystectomy, traction on the gallbladder and dissection in Calot’s triangle can avulse the cystic vein or tear small posterior venous branches, producing bleeding that obscures the critical view of safety and increases the risk of bile duct injury. A posterior perspective also clarifies why venous control can be harder than arterial control in this region, since the vein often lies deeper in connective tissue and may drain variably into the portal vein or right hepatic vein branches.
