Description
Running deep along the posteromedial leg, the posterior tibial vein (venae tibiales posteriores) ascends from the plantar venous plexus and accompanies the posterior tibial artery behind the medial malleolus within the tarsal tunnel. Paired venae comitantes flank the artery and receive tributaries from the posterior compartment, while perforating veins connect the deep system to superficial channels such as the great saphenous vein coursing along the medial leg and thigh. Proximally, the posterior tibial veins converge with the fibular (peroneal) veins and anterior tibial veins to form the popliteal vein posterior to the knee. Clear proximal to distal continuity. Clinically, this course matters because the posterior tibial veins are a common site for distal deep vein thrombosis, and symptoms often localize to the calf despite the thrombus sitting deep to the soleus and gastrocnemius.
