Medial lower leg anatomy is centered on the posterior tibial vein, a paired deep vein (venae comitantes) ascending proximally from the plantar and medial malleolar venous plexuses along the posterior compartment. It runs posterior to the medial border of the tibia and passes deep to the flexor retinaculum at the tarsal tunnel, where it lies with the posterior tibial artery and tibial nerve. More superficially and anterior to the medial malleolus, the great saphenous vein tracks superiorly with its tributaries, separated from the deep system by the deep crural fascia. Perforating veins connect superficial and deep channels along the medial calf. A medial perspective clarifies the key junctional anatomy used every day in venous examination: the relationship of the great saphenous vein to the medial malleolus and the deep posterior tibial vein coursing in the neurovascular plane. This is the pathway interrogated with duplex ultrasound when you suspect distal deep vein thrombosis in the calf, and it also frames how incompetent perforators and reflux in the great saphenous system can feed medial calf varicosities. Small changes in depth matter. The posterior tibial vein is not a surface target. Use this asset in lower-limb vascular anatomy teaching, sonography and DVT modules, and in clinical education materials on chronic venous insufficiency, perforator disease, and endovenous treatment planning that must distinguish superficial from deep venous systems in the male leg.
