Description
Posteriorly, the profunda femoris (deep femoral) artery branches from the femoral artery in the proximal thigh, then courses deep to the adductor longus toward the posterior compartment, where its perforating arteries pass through adductor magnus to reach the hamstrings. Proximally, the medial and lateral circumflex femoral arteries are typically seen arising from the profunda, wrapping around the femur near the intertrochanteric region, with the femoral vein running close to the femoral artery before the vessels dive deeper. Bony landmarks such as the femur and elements of the pelvis orient the vessel to the adductor canal and popliteal region. Deep, fast. This posterior-focused rendering matters because the deep femoral artery is the dominant blood supply to the thigh musculature and a common source of clinically relevant collateral flow when the superficial femoral artery is occluded in peripheral arterial disease. Its perforators sit in the operative field for posterior and medial thigh approaches, and inadvertent injury during hamstring repair, adductor magnus work, or proximal femur surgery can produce brisk hemorrhage that is hard to control in depth. The circumflex femoral branches also tie directly to femoral head perfusion, a teaching point when discussing displaced femoral neck fractures and the risk of avascular necrosis.
