Description
Posterior shoulder anatomy is centered on the deltoid muscle, with emphasis on its spinal (posterior) part forming a thick triangular cap over the glenohumeral joint. Fibers arise from the spine of the scapula and converge inferiorly into the deltoid tuberosity on the lateral humerus (brachium), placing the tendon insertion lateral to the triceps brachii long head. Superiorly, the deltoid wraps around the acromion, while medially it borders the scapular spine and the infraspinous region; laterally it blends into the contour of the proximal arm. Fiber direction is clear. A posterior view matters because the deltoid’s relationship to the quadrangular space and posterior circumflex humeral artery is easiest to teach from this angle, even when the deeper borders are only implied beneath the muscle belly. Clinically, intramuscular deltoid injections are kept high and lateral on the shoulder to reduce risk to the axillary nerve as it courses deep to the deltoid around the surgical neck of the humerus, and this posterior contour helps explain why overly posterior placement can be hazardous.
