Description
Spanning the lateral shoulder, the deltoid muscle (highlighted in blue) is shown as three heads, an anterior (clavicular) part arising from the lateral third of the clavicle, a middle (acromial) part from the acromion, and a posterior (spinal) part from the spine of the scapula. Fibers converge inferiorly into a common tendon inserting on the deltoid tuberosity along the lateral humeral shaft, just distal to the surgical neck. Medially and posteriorly, the trapezius caps the superior scapular region, while the pectoralis major forms the anterior axillary fold and sits anteromedial to the deltoid at the shoulder girdle. Understanding this morphology helps explain why the deltoid contributes differently across shoulder motions, the anterior fibers bias flexion and internal rotation, the middle fibers drive pure abduction, and the posterior fibers extend and externally rotate the arm. Clinically, the deltoid’s relationship to the subacromial space and rotator cuff explains the characteristic weakness pattern after axillary nerve injury, often seen with anterior shoulder dislocation or fractures at the surgical neck of the humerus, where denervation produces deltoid atrophy and loss of the normal rounded contour.
