Description
Posterior to the pelvis, the inferior cluneal nerve courses superficially across the inferior gluteal region toward the cutaneous territory over the lower buttock and gluteal fold. Proximally it arises from the posterior femoral cutaneous nerve in the posterior thigh, then turns superior and medial near the ischial tuberosity, crossing the fascia lata and the inferior border of gluteus maximus. Deep to this plane, the sciatic nerve descends inferior to piriformis and continues distally between the hamstring origins, with the long head of biceps femoris lateral and semitendinosus and semimembranosus more medial. Sacrum and hip bone landmarks (posterior ilium, ischium) set the bony frame. Cutaneous innervation at the gluteal fold sounds minor until it is the pain generator. Inferior cluneal neuralgia is a recognized cause of focal buttock pain, often provoked by sitting and sometimes mistaken for radiculopathy or proximal hamstring tendinopathy because the nerve crosses near the ischial tuberosity and the common hamstring origin. This posterior-inferior angle clarifies where the inferior cluneal branches diverge from the posterior femoral cutaneous nerve and where they can be irritated by scar tissue after hamstring repair, gluteal surgery, or repetitive friction at the subgluteal region.
