Description
Emerging from the sacral plexus on the anterior surface of the piriformis, the inferior gluteal nerve is traced as it exits the pelvis through the greater sciatic foramen inferior to the piriformis and turns posteriorly into the gluteal region. From a posterior orientation, the nerve’s course can be read against the bony frame of the ilium, ischium, and proximal femur, with the hip joint region giving positional context for its entry into the deep plane of the buttock. Nearby neural structures are suggested at the margin of the foramen, where the larger sciatic nerve and posterior femoral cutaneous nerve typically accompany the same gateway. Spatial relationships matter here. Clinically, this view clarifies why inferior gluteal neuropathy is uncommon but well described after posterior hip approaches, deep gluteal injections placed too medial and inferior, or compressive hematoma in the infrapiriform space. Denervation of gluteus maximus produces loss of powerful hip extension and diminished external rotation, a pattern noticed when rising from a chair, climbing stairs, or accelerating from a standing start.
