Description
Arising from the posterior femoral cutaneous nerve, the inferior cluneal nerves curve laterally and superiorly across the inferior margin of gluteus maximus to reach the skin of the lower buttock. Posteriorly, gluteus maximus dominates the superficial gluteal region, with the hamstrings (biceps femoris laterally, semitendinosus and semimembranosus medially) descending inferiorly into the posterior thigh. Deep and slightly medial to this superficial field, the sciatic nerve tracks inferiorly toward the popliteal fossa, while smaller cutaneous branches diverge toward the gluteal fold and proximal posterior thigh. A posterior perspective like this matters because inferior cluneal neuralgia is often mistaken for lumbar radiculopathy or primary hip disease when the true pain generator is entrapment of these cutaneous branches as they traverse the gluteal crease. The relationship to the lower border of gluteus maximus and the adjacent ischial region is a practical landmark during posterior approaches to the hip, gluteal flap elevation, and even routine intramuscular injections placed too inferiorly or medially.
