Description
Posteriorly, the laryngeal mucosa is presented as a continuous lining over the posterior laryngeal wall, flowing superiorly into the hypopharyngeal mucosa and inferiorly into the tracheal mucosa. The paired arytenoid eminences sit superior to the cricoid lamina, with the interarytenoid region spanning the midline between them, and the posterior aspects of the vocal folds converging toward the vocal processes. Inferior to the glottic level, the mucosa transitions into the subglottis and proximal trachea, where the lumen remains centered and cylindrical. A smooth surface dominates. Posterior laryngeal mucosa is where reflux injury and intubation trauma often declare themselves first, with erythema, edema, or contact ulceration around the vocal processes and interarytenoid mucosa in laryngopharyngeal reflux or prolonged endotracheal tube contact. This perspective also clarifies the relationship between the postcricoid region of the pharynx and the laryngeal inlet, a teaching point when discussing swallowing, aspiration risk, and the narrow margin between airway protection and bolus passage.
