Description
Posteriorly, the distal femur and proximal tibia frame the intercondylar region of the knee, with the articular cartilage covering the femoral condyles and the tibial plateau. The posterior meniscofemoral ligament (ligament of Wrisberg) arises from the posterior horn of the lateral meniscus and courses superiorly and medially, running posterior to the posterior cruciate ligament as both approach the medial femoral condyle. Within the intercondylar notch, the PCL forms the dominant central band, while the meniscofemoral ligament parallels it as a thinner, obliquely oriented restraint. Small structure. Big diagnostic implications. Recognition of the posterior meniscofemoral ligament matters because it is frequently mistaken for a displaced meniscal fragment on MRI, classically simulating a bucket-handle tear when seen adjacent to the PCL. This posterior view also clarifies why the ligament can contribute to posterior and rotational stability, sharing load with the PCL and influencing lateral meniscal motion during flexion, a point that becomes relevant in PCL insufficiency and during arthroscopic evaluation of the posterolateral compartment.
