Description
Arising from the upper thoracic and lower cervical transverse processes, the semispinalis capitis forms a broad, obliquely oriented sheet in the posterior neck that ascends superomedially to insert between the superior and inferior nuchal lines of the occipital bone. In this posterior view beneath the skin, it lies deep to the trapezius and splenius capitis and sits medial and slightly superficial to the cervical multifidus, while remaining closely applied to the spinous processes and laminae along the midline. Inferiorly, its fibers blend into the transversospinalis mass above the cervicothoracic junction, with the nuchal ligament occupying the midline interval. Deep extensors dominate the field. Functionally, semispinalis capitis is a principal extensor of the head and a strong contributor to contralateral rotation, so its fiber direction helps explain coupled cervical mechanics during active head turning. This posterior exposure matters in posterior cervical approaches where midline dissection follows the nuchal ligament and subperiosteal elevation proceeds laterally, because preserving muscular attachments and the greater occipital nerve (C2 dorsal ramus) reduces postoperative axial neck pain and scalp dysesthesia. It also clarifies why cervicogenic headache can be driven by C2 irritation where the nerve courses through the semispinalis capitis before emerging near the superior nuchal line.
