Description
Running in the deep posterior neck, the semispinalis cervicis lies medial to the splenius cervicis and splenius capitis and superficial to the multifidus and rotatores of the transversospinalis group. From a posterior perspective, its long fascicles can be followed superiorly toward the cervical spinous processes, with inferior attachments arising from upper thoracic transverse processes and crossing multiple motion segments. Lateral to this column sit the levator scapulae and upper trapezius over the scapular spine and acromial region, while the occipital bone and posterior elements of the cervical vertebrae provide bony landmarks in subdued gray. Semispinalis cervicis matters because it is a principal segmental extensor and contralateral rotator of the cervical spine, and its length and oblique fiber direction explain how it couples extension with subtle rotation during head and neck positioning. Deep posterior cervical pain after whiplash commonly reflects overload of the transversospinalis musculature and adjacent cervical facet joints, and this view helps correlate tenderness or trigger points along the paraspinal gutter with the underlying muscle layers. For posterior cervical approaches, including exposure for C3 to C7 laminectomy or posterior fusion, surgeons separate the midline raphe and reflect superficial extensors to reach the semispinalis cervicis and the laminae while protecting the dorsal rami.
