Description
Posterior cervical dissection highlights the semispinalis cervicis as a deep transversospinalis muscle, positioned medial to splenius cervicis and deep to semispinalis capitis in the upper neck. Its fascicles course superomedially from the transverse processes of the upper thoracic vertebrae (classically T1 to T6) to the spinous processes of mid cervical vertebrae (most often C2 to C5), crossing multiple motion segments. Adjacent landmarks include the cervical spinous processes in the midline, the laminae and facet joint region laterally, and the proximal scapula and clavicle inferiorly at the shoulder girdle. Function follows form. By spanning several vertebrae, semispinalis cervicis generates segmental extension and contralateral rotation while adding posterior stability across the cervical zygapophysial joints, a relationship that becomes clinically relevant in whiplash-associated disorder and chronic axial neck pain where deep extensor deconditioning is common. Surgeons working through a posterior cervical approach for laminectomy or instrumented fusion must identify and respect these deep layers, because excessive detachment from the cervical spinous processes can increase postoperative pain and impair extensor endurance.
