Description
Beneath the posterior skin of the male neck, the splenius cervicis lies deep to trapezius and inferior to splenius capitis, running obliquely superolaterally from the spinous processes of the upper thoracic spine (classically T3 to T6) toward the transverse processes of the mid to upper cervical vertebrae (often C1 to C3 or C4). Its fibers sit lateral to the midline nuchal ligament and medial to the posterior border of levator scapulae as they approach the cervical transverse processes. Superiorly, the occipital region and upper cervical spine provide bony context, while the shoulders frame the scene with superficial contours of the deltoid and the scapular attachment line of trapezius. Deep anatomy is implied, but the surface read remains clinically familiar. Posterior neck pain frequently localizes to this layer. Splenius cervicis strain after whiplash, sustained cervical extension, or prolonged rotation can mimic facet-mediated pain, and myofascial trigger points can refer aching toward the upper cervical region and occiput, blending with headache complaints when adjacent splenius capitis is involved. Palpation and needling or injection planning depend on understanding that trapezius overlies the splenii, while the segmental dorsal rami supply these intrinsic back muscles and travel close to the posterior elements. Depth matters. Use this posterior view for teaching layered dissection in gross anatomy or kinesiology courses, for illustrating cervical extensor mechanics in rehabilitation texts, or for clinical education materials on differential diagnosis of posterior neck pain and targeted manual therapy. It also suits surgical and procedural orientation when describing posterior cervical approaches where superficial reflection of trapezius exposes the splenius plane.
