Description
Posterior neck and upper thoracic anatomy is presented with the splenius cervicis emphasized as paired, obliquely oriented muscle slips lying deep to trapezius and splenius capitis and superficial to the intrinsic back musculature. Fibers arise from the spinous processes of T3 to T6 and course superolaterally to insert on the posterior tubercles of the transverse processes of C1 to C3, placing the muscle lateral to the cervical spinous processes and medial to the scapular region. Superiorly, the occipital bone and the capitis portion frame the more narrow cervicis bundles, while inferiorly the upper thoracic vertebrae anchor the origin. Bony landmarks of the shoulder girdle, including scapula and clavicle, provide regional orientation. Clear separation of splenius cervicis from splenius capitis matters when teaching layered posterior cervical dissection and when correlating focal pain patterns attributed to posterior neck muscles. Splenius cervicis contributes to ipsilateral rotation and lateral flexion of the neck and assists extension when acting bilaterally, so strain can mimic facet-mediated pain in the upper cervical segments. Its relationship to the cervical transverse processes also makes it a useful reference when discussing deep muscle planes encountered in posterior cervical approaches and when differentiating muscular from osseous tenderness on examination. Small muscle, common complaint.
