Description
Arising from the posterior division of the internal iliac artery, the superior gluteal artery courses posteriorly along the lateral pelvic wall and exits the pelvis through the greater sciatic foramen superior to the piriformis. From a posterior perspective, its caliber and branching pattern can be followed deep to gluteus maximus as it divides into superficial and deep branches that track laterally between gluteus medius and gluteus minimus. Bony landmarks including the sacrum, posterior ilium near the posterior superior iliac spine, and the proximal femora frame the vascular pathway, while paired iliac veins run adjacent in blue. Surgeons care about this vessel because it sits in the operative neighborhood of the posterior hip and pelvis. The superior gluteal artery and accompanying superior gluteal vein are at risk during posterior approaches to the acetabulum, posterior column fixation, and iliosacral screw placement, where an errant trajectory can lacerate the pedicle as it traverses the greater sciatic notch and produce rapid retrogluteal hemorrhage. Injury can also compromise perfusion to gluteus medius and minimus and, when combined with superior gluteal nerve trauma, contribute to postoperative abductor weakness and a Trendelenburg gait. A small corridor, little forgiveness.
