Description
Arising from the deep gluteal venous plexus, the superior gluteal vein courses posterior to the hip joint and exits the pelvis through the greater sciatic foramen superior to the piriformis, running with the superior gluteal artery and nerve in the plane between gluteus medius and gluteus minimus. Posteriorly, the vessel lies lateral to the sacrum and sacral plexus and medial to the greater trochanter, with the iliac crest and posterior superior iliac spine providing bony orientation. Portions of the lumbar spine, sacrum, coccyx, ilium, and proximal femora frame the vascular pathway. A deep landmark. This posterior perspective matters because the superior gluteal neurovascular bundle is exposed during posterior and lateral approaches to the hip and during pelvic fracture fixation, where retraction near the greater sciatic notch can injure the vein and produce brisk bleeding that is hard to control in the confined gluteal compartment. The same anatomy explains the risk of superior gluteal artery pseudoaneurysm or arteriovenous fistula after intramuscular injections placed too medially or too inferiorly in the buttock, with the vein acting as the low-pressure outflow conduit. Seeing the vein in situ, relative to the sacral plexus and piriformis level, clarifies why “safe” injections target the superolateral quadrant.
