Description

Arising from the deep gluteal region, the superior gluteal vein is shown coursing laterally above piriformis, accompanying the superior gluteal artery as both exit the pelvis through the greater sciatic foramen. From this lateral perspective, the vessel sits posterior to the acetabulum and hip joint, then runs between gluteus medius and gluteus minimus toward its drainage into the internal iliac venous system. The os coxae, sacrum, lower lumbar vertebrae, and proximal femur frame the pathway, while adjacent neurovascular elements of the superior gluteal bundle lie in close apposition. Clear relationships matter here. Surgical and interventional relevance centers on the deep location and short extrapelvic course of the superior gluteal vessels. During posterior pelvic approaches, acetabular fracture fixation, or buttock injections placed too high and deep, the superior gluteal vein and artery can be injured where they traverse the greater sciatic foramen, producing brisk hemorrhage and a challenging field. Gluteal compartment syndrome and post-traumatic pseudoaneurysm of the superior gluteal artery are usually discussed, but venous injury and hematoma tracking along the fascial planes between gluteus medius and minimus is the practical companion problem, and this view clarifies the route.
