Description
From a superior vantage over the upper abdomen, the hepatobiliary lymphatics track alongside the extrahepatic biliary tree, clustering around the common bile duct as it descends toward the duodenum. Lymphatic vessels course within the hepatoduodenal ligament, running parallel to the portal triad, so the common bile duct lies anterolateral to the portal vein with the proper hepatic artery positioned anteromedially. Small pericholedochal and cystic duct channels converge toward regional lymph nodes at the porta hepatis, then continue posteriorly toward retropancreatic and celiac pathways. Relationships are tight. Millimeters matter. This superior perspective matters because it mirrors the operative corridor used in cholecystectomy and hilar dissection, where nodal basins along the common bile duct, hepatic artery, and portal vein influence both staging and the risk of bile duct injury. Pericholedochal and hepatic hilum nodes frequently enlarge in cholangitis and obstructive jaundice, and they represent common routes for lymphatic spread from gallbladder carcinoma and perihilar cholangiocarcinoma. The view also clarifies why pancreatic head and retroduodenal inflammation can confound hepatobiliary nodal assessment, given shared drainage toward retropancreatic stations.
