Seen from a superior perspective, the laryngeal inlet opens centrally with the paired vocal folds forming the glottis in the midline, their free edges extending anteroposteriorly from the thyroid cartilage region toward the arytenoid cartilages posteriorly. Posterior to the vocal folds, the arytenoid cartilages sit on the superior margin of the cricoid lamina, with the interarytenoid region spanning between them and the aryepiglottic folds forming the lateral boundaries of the aditus. Intrinsic laryngeal muscles logically frame these cartilaginous landmarks: fibers of the posterior and lateral cricoarytenoid occupy the posterolateral and lateral aspects of the cricoid, while thyroarytenoid (including the vocalis) runs parallel and lateral to the vocal ligament within the vocal fold itself. Orientation is unambiguous. This superior view matters because it maps muscle action directly onto airway caliber and phonation. Abduction at the vocal processes, driven by the posterior cricoarytenoid, widens the rima glottidis and is the movement most threatened in bilateral recurrent laryngeal nerve palsy, where cords rest near the midline and stridor dominates the presentation. Adduction and fine tensioning, coordinated by lateral cricoarytenoid, interarytenoid, and the thyroarytenoid vocalis, explains why small arytenoid malposition or post-intubation cricoarytenoid joint fixation can produce a breathy, unstable voice without obvious supraglottic obstruction.
