Description
Viewed from superior, the L5 vertebra presents a broad vertebral body anteriorly with a centrally placed vertebral foramen framed by stout pedicles that bridge posterolaterally into the vertebral arch. Laterally, the transverse processes extend from the junction of pedicle and lamina, while the superior articular processes sit posterolateral to the foramen, oriented to meet L4 at the zygapophyseal joints. Posterior elements are foreshortened from this angle, but the laminae converge toward the base of the spinous process. Thick pedicles dominate the silhouette. L5 is a mechanical and surgical crossroads between the lumbar spine and sacrum, and the superior view makes the canal, pedicles, and facet orientation easy to judge in one glance. Pedicle morphology at L5 matters for pedicle screw fixation, where a medial breach risks the thecal sac and cauda equina in the vertebral canal, and an inferior or anterior breach can endanger the L5 nerve root as it courses toward the L5 to S1 foramen. Facet alignment at this level also sets the stage for degenerative spondylolisthesis and pars interarticularis stress injury, conditions often discussed alongside L5 to S1 disc disease.
