Description
Centered in the frame, the L4 vertebra is presented from an anterior perspective with a slight superior tilt, bringing the superior endplate and the breadth of the vertebral body into clear relief. The vertebral body sits anterior to the vertebral foramen, with the pedicles forming the posterolateral margins that lead back toward the laminae and spinous process, which remain largely out of view from this angle. Transverse processes extend laterally from the vertebral arch, and shallow vascular grooves and nutrient foramina interrupt the cortical surface. A stout lumbar build. Anterior views of L4 matter because this segment commonly anchors level identification in lumbar surgery and imaging, lying near the iliac crest reference line used during palpation and procedural planning. The broad vertebral body and endplate contours are also where degenerative change becomes clinically visible, including anterior osteophytes and endplate sclerosis seen in lumbar spondylosis, and the vertebral body is the target structure for lateral approaches and vertebroplasty planning after compression fracture. The relationship of the body to the pedicles is a key teaching point when correlating AP radiographs and CT with pedicle screw trajectories, even if the posterior elements are not emphasized here.
